Welcome to the New Mexico Association for Continuity of Care

NMACC is a non-profit organization dedicated to promoting the concept that continuity of care is an essential component of the health care delivery system. For over 20 years NMACC has been a resource in our community that believes in the concept that professionals responsible for continuity of care should have access to an educational and supportive network.

Our main state chapter is located in Albuquerque, New Mexico and holds monthly meetings. Our main objectives focus on defining coordinated planning as an essential component of continuity of care and providing quality educational presentations that serve the needs and professional growth of the members responsible for continuity of care.

We have exciting news for your family members and staff providing caregiving services for our elder population. NMACC is the sole sponsor for the Eldercare Series provided at CNM. The series has five classes, each held on a Saturday morning this April and May.
